There is a phrase we hear a lot in clinic, almost always said with a mix of resignation and humour: “Well, I’m in my forties now, I suppose it’s normal.” And it almost always comes with the same picture: neck pain that has been there for months, shoulders and neck that never quite loosen up, and a head that by the end of the day feels twice as heavy as it should.
We want to talk about that. Not to alarm you, but to share what we see every week in our clinic: that many people have been normalising something that has a solution for far too long.
When a loaded neck becomes the new normal
By 40, the body has been accumulating load for years. Hours in front of a screen. Tense meetings. Long working days where movement is barely a thought. A phone we look down at more than we realise.
All of that leaves a mark. And the cervical region is one of the first areas to feel it.
What happens is gradual. First it is a tension that appears at the end of the day and disappears with rest. Then that tension starts staying longer. Then it becomes morning stiffness. And at some point, without any specific incident having occurred, the neck is always tight.
That is when many people normalise it. “It’s my job.” “It’s the stress.” “That’s just how I am.”
But it does not have to be that way.
Before the digital era, cervical degeneration appeared in people over 65. Today, the average starts from 40 to 45. The main cause is not age: it is sustained overuse of the cervical muscles combined with too little movement and too much stress. And unlike age, that has a solution.
What is behind the pain you have been ignoring
Neck pain does not appear for no reason. It has its own logic, and understanding it helps a great deal.
When we spend many hours in the same position: head slightly forward, shoulders raised, jaw clenched without realising it, the cervical muscles work continuously to support the weight of the head. In a neutral position, the head weighs around five kilograms. When it projects forward, that load can multiply by four or five times on the neck muscles.
Sustained over hours, weeks and months, that generates a cervical contracture that does not go away on its own. The muscles shorten. Mobility decreases. And the pain that was at first occasional becomes constant.
Stress amplifies all of this. When the nervous system is under sustained activation, the trapezius and neck muscles maintain a higher tone than necessary.
This is not a perception: it is a real physiological response that can be measured. The body prepares for something that never comes, and that permanent muscular readiness exhausts the tissues.
And then there is the jaw. Many people who have been carrying neck tension for some time start to notice they clench their teeth at night, wake up with a heavy jaw, or feel pressure in the temporomandibular joint.
That is not a coincidence. The cervical region and the jaw are closely connected. When one is under load, the other often compensates.
Tension headaches are another frequent consequence. That headache that starts at the base of the skull and travels up toward the forehead, appearing at the end of the day or on waking in the morning, very often has its origin in the cervical muscles and not in another cause.
The signs that tell you it is time to act
We do not want to sound alarmist. Most neck pain is not serious. But there is an important difference between occasional pain that resolves with rest and pain that has become part of your daily routine.
There are some signs that, in our experience, indicate that it makes sense to ask for a professional assessment.
If neck pain has been present for more than three weeks without improving significantly, your body is telling you it will not resolve this on its own. If you notice that your neck mobility has decreased, that turning your head to one side is not as easy as it used to be, there is more than occasional muscle tension involved.
If the pain appears regularly at the end of each working day, if it comes with frequent headaches or that feeling that the shoulders are always raised, the pattern is too well established to expect it to disappear by itself.
And if you notice tingling or a sensation of numbness travelling down toward the arm or hand, do not leave it. That can indicate that a nerve structure deserves attention.
What we want to be clear about is this: living with recurring neck pain is not an inevitable consequence of being over 40 and working hard. It is a signal that deserves attention. And the sooner it is attended to, the simpler it is to resolve.
What a complete first session can reveal
At Fisio Physio Clinic Salinas, when someone comes in with neck pain that has been established for some time, we dedicate the entire first session to understanding what is happening. It lasts more than an hour. And that is not a minor detail.
We start with the clinical history. We want to know what your working day looks like, how many hours you spend in front of a screen, whether you drive a lot, whether there are periods of more stress than others.
We want to know when the pain appeared, how it has evolved, what makes it better and what makes it worse. Whether you have had previous episodes. Whether you are taking anything to manage it.
All of that tells us a great deal before we examine anything.
Then comes the assessment. We evaluate cervical mobility across all its ranges. We review muscle tension, identify active trigger points that may be radiating pain toward the head or arm.
We assess overall posture: whether there is a shoulder rotation, whether the head is positioned forward relative to the body’s axis, whether something in the thoracic region is generating compensations higher up.
And with all that information, we design a concrete treatment plan for your case. Not a generic physiotherapy protocol for neck pain. A plan built around you, around what your body is doing and around the goals that make sense in your situation.
Sometimes treatment begins in that very first session, when the clinical picture allows it. And almost always, the patient leaves that first visit understanding much better what is happening in their body. That, in itself, is already worth a great deal.
Why a personalised approach changes the outcome
There is something we have learned in more than twenty years working at a physiotherapy clinic in Alicante: no two cases of neck pain are the same, even when patients describe them in similar terms.
The neck pain of someone who works eight hours a day in front of a screen in a static position is not the same as that of someone who drives many hours, nor that of someone who trains intensely but neglects their posture.
The causes are different, the tissues involved are different, and the treatment needs to be different too.
When the same protocol is applied to everyone, massage, heat, electrostimulation, generic stretching, the results are partial and temporary. The pain improves for a few weeks and comes back, because what was generating it remains untouched.
