When someone comes to a chiropractic and physiotherapy centre for the first time, they usually arrive with a mix of curiosity and scepticism. We find that completely reasonable. 

Chiropractic has a somewhat blurry public image. For some people it is that neck-cracking thing that goes around in videos online. For others it is an alternative therapy of questionable credibility. And for others it is simply something “someone recommended” without them quite understanding what it involves. 

We want to try and clear that up. We believe the best way for someone to make an informed decision about their health is to understand what they are dealing with. 

What chiropractic is, and what it is not 

Chiropractic is a discipline within the musculoskeletal field. It focuses on the relationship between the spinal column and the nervous system. Its main tool is the vertebral adjustment: a precise manual technique applied to the joints of the spine. The aim is to restore their range of movement and reduce pressure on the adjacent nerve structures. 

It is endorsed by the World Health Organisation. It requires specific university training of between five and seven years. And it has support in systematic reviews published in leading scientific journals for the treatment of lumbar and cervical pain. 

What chiropractic is not: it is not a miracle treatment that resolves any pain in a single session. It is not a massage. It is not dangerous when applied by a qualified professional. And it does not create dependency. 

A chiropractic adjustment also does not involve “putting bones back in place”. What it does is work on the articular mobility of segments that have lost it, using techniques adapted to each person and each case. 

What actually happens during a vertebral adjustment 

The sound is the first thing that catches the attention of anyone who has never received a chiropractic adjustment. That crack raises more questions than it answers. 

What happens is a phenomenon called articular cavitation. When the joint recovers its mobility through spinal manipulation, the change in pressure releases gas bubbles from the synovial fluid surrounding it. The sound does not indicate that anything has been “put in place” or “knocked out of place”. It is simply a consequence of joint movement, the same as when someone cracks their knuckles. 

And it is worth saying clearly: there is not always a sound. There are many chiropractic adjustment techniques. From the more structural ones to very low-amplitude approaches, or those using specific instruments. Some are completely silent and the patient experiences them as a gentle, precise movement with no discomfort at all. 

What does happen in every case, when the adjustment is well indicated, is a reduction in pressure on the nerve roots. Also an improvement in the articular range of movement in the treated area. And a nervous system response that can translate into a reduction in perceived pain. 

The mechanisms of action include direct biomechanical effects and neurophysiological modulation. That is, the way the nervous system regulates pain perception following the mechanical stimulus. 

The complaints where it is most commonly sought 

Not every complaint responds equally to chiropractic. An important part of the work in clinic is assessing whether it makes sense to propose it. That said, there are clinical pictures where the evidence and clinical experience are consistent. 

Lower back pain is the area where chiropractic has the most scientific support. Cochrane reviews have concluded that spinal manipulation produces significant improvements in acute and chronic lower back pain. In some studies, results are comparable to first-line treatments. 

It is particularly useful when there are articular blockages in the lumbar or sacroiliac region. Also when the pain comes with marked stiffness, or when conventional treatments have only given partial results. 

Cervical pain is another picture where chiropractic shows solid results. Especially when there are mobility restrictions in the cervical segments generating local pain, cervicogenic headache or a persistent sense of stiffness in the neck and upper back. 

Sciatica is perhaps the most expectation-laden reason for consultation. And here it is important to be precise. Chiropractic can be a useful tool when sciatica has a clear mechanical component: compression from articular blockage, piriformis muscle tension, or irritation from sustained posture. 

But it does not resolve a severe herniated disc with neurological deficit on its own. Prior assessment is essential to determine whether it is a suitable option or whether the case requires a different approach. 

General articular stiffness is also a frequent reason for consultation at a chiropractic and physiotherapy centre. That feeling that the back “has nothing more to give”, blockages that appear after an effort or simply on getting up in the morning. In these cases, the vertebral adjustment can restore range of movement with a speed that other techniques do not always match. 

Posture, movement and habits: what no adjustment can replace 

One of the most common mistakes is presenting the chiropractic adjustment as if it were enough on its own. As if freeing an articular blockage resolved the underlying problem. 

It is not. And saying so clearly is not a limitation of the treatment: it is clinical honesty. 

Back pain, neck pain or articular pain rarely has a single cause. Behind it there is almost always a combination of factors: posture, movement habits, level of physical activity, quality of rest and accumulated muscle tension. In many cases, also sustained stress. 

A chiropractic adjustment can release the blockage that is generating pain at that moment. But if the person returns to the same conditions that generated it, the blockage will come back. 

Chiropractic done well is not a visit to the mechanic to have something broken fixed. It is an intervention on the articular and nervous system that has more impact when accompanied by real changes. Improving workstation ergonomics. Incorporating regular movement. Strengthening the musculature surrounding the spine. Improving sleep quality. 

Without that context, the relief may be real but temporary. 

That is why at Fisio Physio Clinic Salinas we do not present chiropractic as a standalone service. We integrate it within a broader view of each patient’s case. Because the chiropractic adjustment opens a window of opportunity, and that window needs to be taken advantage of with complementary work. 

Realistic expectations: what you can and cannot expect 

This section is one of the most important in the article. And also one of the least written about, probably because it is not the most appealing thing to read. 

What you can expect: an improvement in articular mobility that in many cases is noticeable from the first sessions. A reduction in pain that can be significant, especially in acute or subacute cases with a clear mechanical component. A sense of relief and greater ease of movement that many patients describe as liberating. 

What you should not expect: that one session resolves a problem of months or years. That chiropractic corrects on its own herniated discs with neurological deficit, fractures or active inflammatory processes. That the effect of the adjustment is permanent if it is not accompanied by changes in the factors that generated the problem. 

The number of sessions varies greatly depending on the case. Some people with an acute blockage improve noticeably in two or three adjustments. More complex chronic cases need a longer and more combined process. 

The important thing is that in a good chiropractic and physiotherapy centre this planning is done on an individual basis. With clear objectives. And with honesty about what can be expected at each stage. 

It is also normal to feel slight muscle soreness in the hours after the first adjustment. Similar to how you feel after exercise. It is temporary and goes away within a day. What should not happen, is that the treatment generates more pain than there was before. 

The role of chiropractic within a broader plan 

Chiropractic works best when it does not work alone. This is one of the things we feel most strongly about at Fisio Physio Clinic Salinas, and it makes a real difference to the results our patients achieve. 

When chiropractic and physiotherapy are combined in a coordinated way, each discipline contributes what the other cannot do as well. 

The chiropractic adjustment is very effective at restoring articular mobility. At reducing pressure on nerve structures. At breaking the blockage-pain-tension cycle that keeps many chronic cases going. 

Physiotherapy, for its part, works the musculature surrounding those recovered joints. Re- educating movement. Strengthening what needs to be strengthened so that the improvement lasts. 

Without the muscular and postural work that physiotherapy provides, the chiropractic adjustment can lose effect sooner than desirable. The soft tissues return to the patterns they knew. Without the articular release that the adjustment provides, physiotherapy work meets more resistance than necessary. 

When adapted therapeutic exercise is added to that combination, the result is a plan that does not just relieve pain. It builds a more solid foundation so that it does not return with the same frequency. 

In a well-structured chiropractic and physiotherapy centre, these disciplines are not independent services the patient chooses separately. They are tools the team coordinates according to the case and the stage of the process. That is what sets a serious approach apart from one that simply applies techniques without an overall guiding principle. 

The best care is not the most dramatic 

Some centres have a tendency to present chiropractic with very impactful language. Transformative adjustments. Liberating cracks. Guaranteed immediate results. 

We understand the appeal of that kind of language. But it is not the language we use, because it does not do anyone any favours. 

The best chiropractic care is not the most spectacular. It is the care that starts by listening and assessing properly. That determines whether the vertebral adjustment is the right tool for your specific case or whether another approach makes more sense. That explains clearly what you can expect and over what timeframe. And that does not propose more sessions than necessary.
