First physio session: what happens and how it can help you

The clinical history is the starting point of any serious assessment. We want to know when the pain began, how it has evolved, what makes it better and what makes it worse. Whether there are periods when it intensifies. Whether you have had previous injuries, even old ones. Whether you have received any prior treatment and how you responded. 

We also ask about your life context: how many hours you spend sitting, whether your work generates a lot of stress, how you sleep, whether you do sport and what kind. That information is not secondary. It is fundamental.

The muscular pain from stress of someone who works twelve hours a day in front of a screen is not approached in the same way as that of someone who has had a recent sports injury, even if both arrive with a loaded neck. 

Everything you tell us in this first part guides the assessment that follows. Without that context, the physical examination has far less value. 

The assessment: what the body says without words 

Once we have the clinical history, we move to the physical examination. And this is where clinical physiotherapy offers something that no imaging test can provide: a direct reading of how your body is moving right now. 

We evaluate articular mobility in the affected area and in related areas. We review muscle tension, look for active trigger points that may be radiating pain toward other zones. We assess overall posture: whether one shoulder sits higher than the other, whether the head is positioned forward relative to the body’s axis, whether there is any pelvic rotation generating compensations higher or lower. 

We also look for asymmetries between one side of the body and the other. Very often the origin of the problem is not where it hurts. It is in another area that has not been moving well for some time and has gradually been overloading adjacent structures until one of them reached its limit and started complaining. 

That reading of movement is what allows us to do something we consider essential at our physiotherapy clinic: identify the mechanism behind the pain, not just the point where it is located. 

The treatment plan: personalised from day one 

With the clinical history and assessment complete, we have enough information to explain what we found and propose a concrete treatment plan. 

Not a generic protocol. Not “ten physiotherapy sessions for the neck”. A plan built around you, around what is happening in your body, around your lifestyle and your goals. 

That plan may include direct manual work on the tissues, progressive therapeutic exercise, postural re-education, guidance on ergonomics or movement habits, or a combination of several things. The proportion and order depend on what we found in the assessment. 

And very often, treatment begins in that very first session. When the clinical picture allows it, we do not wait until the second visit to do something useful. Leaving the first session at the physiotherapy clinic having already received therapeutic work, not just a cold assessment, makes a real difference to how the patient experiences that first contact. 

We also explain, clearly and without jargon, what we found and why the proposed approach makes sense. Understanding your own pain changes your relationship with it. And in many cases, that understanding is already part of the treatment.
